AlphaTrace Request Form Guidelines

• We ask that your firm please attach any available documents including but not limited to: TCR, LOA, insurance card, driver's license, photos and other claim related documents.

• To expedite processing, include as much information as possible about both defendant and policyholder.

• Scroll down using mouse scroll wheel or scroll bar on right of form.

• In the event a duplicate request is submitted, client will not be double charged.

• All requests submitted are processed under a "no hit, no fee" basis, meaning your firm will not be invoiced unless we successfully obtain policy information requested.

• Trace results will be emailed in PDF format to the requester's email provided on the submission form.
